Why a QR code feedback form works for hospitality
Most happy customers never leave a review, and unhappy ones often leave it somewhere public. A short form that's one scan away at the table gives every guest an easy, private way to tell you how it went — so you hear about the cold food, slow service or stand-out server while you can still do something about it.
Example questions
Describe your goal as something like "how was your visit today?" and choose a short form. You might get:
- How was your experience today? (1–5 rating)
- What stood out most? (Service · Food & drink · Atmosphere · Value · Speed)
- Is there anything we could have done better? (open text)
Edit any of them before publishing. Try the live example café form to see exactly what guests see.
Where to put the QR code
- Table tents or stickers — guests can answer while waiting for the bill.
- Printed receipts — "Tell us how we did" with the QR code at the bottom.
- Menus and takeaway packaging — catches delivery and takeaway customers too.
- By the till or door for counter-service cafés.
Run more than one form
Use separate forms for different questions — one for general visits, one for a new menu, one for a catering service — and filter your results by form. Several locations? Add each as its own brand; one Pro subscription covers all of them.
Turn comments into actions
Ratings and option counts are summarised automatically. With Pro, AI response insights read every comment and tell you which issue comes up most often — for example "wait times at lunch" — with how many people mentioned it, a real quote and a suggested fix. Online mention tracking adds what people are saying about you elsewhere on the web.
